Your custom roadmap
We’ve developed a meticulously crafted, comprehensive six-step process that guides you to your custom power wheelchair with clarity and ease.

Face to Face Visit with your Healthcare Provider
The custom mobility process begins with a Face to Face visit with your healthcare provider who can determine if a powered mobility device may be appropriate for you to reach your daily living and independence goals.
02.
Personalized Mobility and Seating Assessment
Alongside your healthcare provider, we develop a comprehensive understanding of your history, goals, and lifestyle. A Crest Assistive Technology Professional (ATP) will begin designing a chair that is right for you in partnership with a Physical or Occupational Therapist on your care team.
03.
Prior Authorization Request
The Crest Mobility team has decades of experience successfully navigating the insurance approval process. We handle the process for you of collecting the required medical necessity documentation from your healthcare team and submitting a prior authorization request to your insurer.
04.
Building your Custom Power Wheelchair
Once your insurer has given approval, the team at Crest orders the components custom to you and builds your chair with expertise.
05.
Personalized Fitting and Education
At your convenience, a Crest ATP will deliver your completed chair to you where the chair will be fitted specifically to your needs with adjustments being made as needed. You will also be guided through the operation of the chair and its systems, so you can feel comfortable and knowledgeable.
06.
Continued Support
After the delivery of your chair, the Crest Mobility team will continue to support your mobility needs. Our technicians and ATPs are here to assist as needed.
